Hello Families, If your scholar still needs a sports physical, a clinic provides sports physicals; insurance is not required. Below is the information for the clinic. The Free Walk-In Clinic is open Mondays from 4-7 p.m. It is at our Lifehouse Forest Park campus- 2100 Kentucky Avenue, Fort Wayne, 46805. Enter through Door #1. No appointments are necessary. Scholars can still participate in basketball or cheer even if they missed tryouts, but they do need a physical to participate. That physical form needs to be brought to the school for our records.
